Solismed was created as an alternative to costly EHR systems for solo practitioners and small-scale clinics. So far, they've outperformed expectations. They provide an entirely free version that will cost you nothing - except your time - if you prefer to put it up yourself. The cherry on top is that it's open-source, which means they can't sell your medical data because their code is visible to everyone.
